Pet Identification

At Northwest Animal Hospital we know that proper pet identification can prevent heartache and save lives!  That's why we established the Pet Identification Center, which you can find in our lobby. We offer many options from which to choose, including inexpensive pet tags that list vital information about your pet, and microchips, which are small rice-sized computer chips that can be read by a scanner if your pet becomes lost. Microchips are placed on your pet during a routine visit or during surgery, such as when a pet is spayed or neutered.

It only takes a few moments to complete the information you want to put on your pet's tag, and in many cases you can do it while you wait. Or you can download the Pet Identification Center form and send or fax it to us, and we will be happy to send your custom-made tag to you through the mail.

But remember, for best protection we recommend that you provide your pet with both a pet tag and with the microchip. It is so easy—and you may save your pets life!
